Chrysalis is a company that provides a variety of support services for at-risk adults and/or juveniles that have developmental and intellectual disabilities.  These individuals also have a variety of fragile medical issues.  This position requires the ability to work in stress-filled and safety-sensitive environments, make crucial decisions in regards to the care and treatment of at-risk individuals, and possess the ability to communicate those decisions effectively to others.  The Supported Employment Specialist Manager will supervise all employment staff and will be responsible to supervise clients while continuing their education and employment in the community. Assist with the tasks that are assigned to the clients in order to keep the ongoing progress with them furthering their independent skills in the community. Fill out assessments of how clients are doing with the tasks assigned and complete attendance with the when we are assisting them.

Essential Duties and Responsibilities:

  • Communicate effectively to the individuals
  • Punctuality and regular/consistent attendance at the worksite are required and the employee must be able adhere to their workplace schedule in order to ensure proper oversight of individuals.   
  • Stay awake, responsive, and communicate effectively to the individuals
  • Conduct interviews
  • supervise all employment team members and conduct plan of action plans
  • Build relationships with employers
  • Teach individuals daily living skills
  • Manage employment calendars
  • Attend team meetings for clients
  • Prepare ACRE correlation agenda
  • Attend VR meetings
  • Daily monitoring of employer needs
  • Plan appropriate activities for the individuals
  • Create opportunities for people to increase quality of life
  • Complete the USOR 93 form for each individual monthly
  • Update Employment Support Plans
  • Submit timesheets on time
  • Attends CRP meetings through VR
  • Ensure that all authorizations are on time and accurate per the Individualized Plan of Employment with the VR team
  • Work with DWS for special classes
  • Track hours
  • Coordinate with Job Coaches and Managers
  • Audit and upload VR summaries into fusion correctly and on time
  • Request Job Coaching Authorizations
  • Complete lump sum authorization forms to justify billing
  • Support the individual’s choices
  • Provide a clean, healthy, and safe environment
  • Implement Person Centered Plans
  • Supervise individuals according to the Supervision Guidelines
  • Follow Behavioral Plans
  • Covering Job Coach shifts
  • Assist and supervise individuals with taking the appropriate medication
  • Complete necessary administrative tasks within established time frames
  • Complete and document all documentation required by Chrysalis and the State
  • Maintain communication with the Manager regarding the individual’s needs
  • Track and submit pay statements to subsidies
  • updating LSVR, monitor transition to DSPD for services as they are exhausted
  • Coordinate CE hours with employment team per direction from USOR
  • Coordinate with employers to complete and submit DWS Employment Information and Termination forms
  • Be available after regular hours to answer texts and calls regarding the program needs
  • Other duties as assigned by the supervisor

Requirements 

  • Minimum of least 21 years old
  • Pass and maintain a passable criminal background check and LEIE per state regulations
  • Be eligible to drive according to Chrysalis policy
  • Should possess the ability to speak, read and write effectively in English

Experience or Education:                              

  • High school diploma or GED

Training and Certifications:

  • Certified Supported Employment Pathways (SE)
  • Certified Customized Employment Pathways (CE)
  • Certified through Association of Community Rehabilitation Educator (ACRE) Employment Services Basic Level  (within 6 months of hire)
  • Complete all initial and on-going training requirements within established time frames
  • Complete annual CPR, First Aid, and OSHA training (All Employees)
  • Obtain and maintain SOAR certification (All Employees)

Essential Knowledge, Skills, and Abilities:

  • Understand policies and follow the Provider Code of Conduct/ Abuse Neglect and Exploitation Policy according to state standards
  • Excellent public relations skills working with the individuals’ family
  • Ability to communicate effectively with medical, professional, and state employees
  • Demonstrate good judgment and ability to handle crisis situations
  • Have excellent interpersonal skills working with the individual and co-workers
  • Demonstrate a service oriented attitude
